APOE Genetic Test Kit Condition of Gift Agreement

By requesting the APOE genetic test kit gift, the requestor acknowledges and agrees to the following terms:

1. Courtesy Provision: The APOE genetic test kit is provided as a courtesy by The Dementia League. We make no guarantees regarding the accuracy, reliability, or outcomes of this test kit. However, the test kit has an accuracy rate of 99.99%.
    
2. Third-Party Laboratory: The Dementia League partners with Kashi Laboratories for this genetic assessment. The Dementia League prepays for this assessment, and Kashi Laboratories is responsible for processing and delivering results.
    
3. Results & Interpretation: Upon completing the test, Kashi Laboratories will mail the requestor an informational packet containing:
    
   • Test results
   • A guide on how to interpret the results
   • Support Number

1. No Support or Consultation: The Dementia League does not provide any consultation or support regarding test results. Please do not contact The Dementia League with questions about your results. Any inquiries should be directed to a qualified medical professional or genetic counselor.
    
2. Liability Waiver: By accepting and using this test kit, the requestor will not hold The Dementia League liable for any results, lack of accuracy, or interpretation of the test.  Additionally, the requestor acknowledges that test results may cause emotional discomfort, and The Dementia League holds no responsibility for any distress experienced.
    
3. Acceptance of Terms: By requesting and using the APOE genetic test kit, the requestor understands and agrees to the terms outlined in this agreement.
